宿泊者情報のご登録Guest Registration Form

旅館業法／住宅宿泊事業法により、宿泊者名簿の作成が義務づけられています。ご宿泊されるお客様全員分のご記入をお願いいたします。ご到着前のご登録にご協力ください。 Under Japanese law (Hotel Business Act / Private Lodging Business Act), we are required to keep a guest register. Please complete this form for every person staying, before your arrival.

2. 宿泊者情報（全員分）2. Guest details (all guests)

法令に基づき、宿泊されるお客様全員の氏名・住所・連絡先・職業のご記入が必要です。日本国内に住所のない外国籍のお客様は、国籍・旅券番号・旅券（顔写真ページ）の画像もあわせてご提出ください。 By law we must record the name, address, contact and occupation of every guest. Foreign guests without an address in Japan must also provide nationality, passport number and a photo of the passport (photo page).

個人情報の取扱いについてHow we handle your personal data
  • 利用目的：宿泊者名簿の作成・保存（旅館業法第6条／住宅宿泊事業法第8条）、ご滞在中のご連絡、緊急時対応、宿泊統計の報告。
  • 保存期間：宿泊者名簿は作成の日から3年間保存します。
  • 第三者提供：法令に基づく場合（保健所・警察等の求め）を除き、ご本人の同意なく第三者に提供しません。
  • 旅券画像は本人確認および名簿保存の目的にのみ使用します。
  • お問い合わせ窓口：
  • Purpose: to create and keep the legally required guest register (Hotel Business Act Art.6 / Private Lodging Business Act Art.8), to contact you during your stay, for emergencies, and for statutory statistics.
  • Retention: the guest register is kept for 3 years from the date of creation.
  • Disclosure: never shared with third parties without your consent, except where required by law (public health centre, police, etc.).
  • Passport images are used only for identity verification and the guest register.
